To help us get started and ensure we provide the most appropriate support and allocate the appropriate practitioner, please feel free to send through the following information at your earliest convenience:
- Participant’s location, date of birth and NDIS number.
- NDIS goals related to behaviour, independence, or daily living.
- Diagnoses and key behavioural concerns.
- Current Behaviour Support Plan.
- Functional Behaviour Assessment .
- Relevant medical or psychological reports.
- Current list of prescribed medications.
- Allied health reports (e.g., OT, speech pathology).
- School reports or progress notes (if applicable).
- Any available ABC data or incident records.
- Any other relevant documents.
Providing this information at the referral stage helps understand the individual needs, review any existing assessments, and ensure the support we provide is timely, appropriate, and aligned with NDIS safeguarding requirements. It also allows us to collaborate effectively with the participant’s broader support team and develop a tailored, person-centred plan from the outset.
